Can two separate flange valves be considered to provide as a seal in the pipeline?

Actually, the valve flange-end that is considered the sealing face is part of the valve body (flanged or flangeless valves) and so the separate flanges are there to connect the valves-body, they are not required to seal. To consider sealing, there are flanged end connections such as flat face, raised face, and ring type joint.

No, never hear of this issue. Until and unless the operable conditions are above the designed conditions of the flanges, this should not happen.
